Each of our residents comes to us with very different and specific rehab needs. Our entire rehabilitation team is committed to create individualized therapy programs that directly answer those specific needs. We encourage the resident to achieve their maximum outcomes of independence and ability.
Case management works together with the resident, family members, physicians and all clinical disciplines to ensure each resident's needs are met throughout their stay, and ensures a seamless return to the community. Case managers are experienced nurses who have been specially trained. They assist our residents throughout the continuum of care by:
- coordinating the interdisciplinary team to ensure the individualized plan of care is met
- providing oversight to make certain discharge goals and target lengths of stay are met
- assessing and arranging for all post-discharge needs:
- Home care services
- Outpatient therapy
- Durable medical equipment
Balance, variety and moderation are the keys to healthy eating. Our nutrition services professionals are committed to providing exceptional services to meet your personal nutrition goals.
Registered Dieticians (RD) and Dietetic Technician Registered (DTR) complete a comprehensive nutrition assessment for each resident. Following the assessment, a customized nutrition plan of care is developed based on the physician prescribed diet order, individual nutrition needs, lifestyle and medical history. Nutrition counseling is provided for a variety of therapeutic diets and healthy food choices.
The Nutrition Services team works closely with the Food Services Department to ensure that residents receive the appropriate foods based on individual needs and preferences. Selective menus are available for unrestricted and diets prescribed for a variety of medical conditions. We also work closely with the Speech Therapy department to provide the appropriate modified textures for residents with difficulty swallowing.
The social workers at Bethel Health Care and Cascades Assisted Living interview clients upon admission to determine a plan of care that addresses the strengths of our clients and opportunities to assist in areas of psychosocial concerns. Our social workers come with extensive experience in healthcare, are licensed and available to address any questions from resident or family members. They embrace the concepts of Planetree that aim for a resident/patient-centered approach. Through annual training, Resident Rights are reviewed with clients, family members and staff. If an individual requires long-term care, our social workers become a critical discipline in assisting clients and their families through that transition.
A positive and uplifting atmosphere is vital to the success of healing, recovery and rehabilitation. Bethel Health and Rehabilitation Center employs outgoing and friendly professional staff to create social programs and activities to fit the residents needs. We provide a monthly activity calendar to each resident to keep them informed of upcoming events. Friends and families are always welcome to visit and attend our special events.
